Fix the System, Not the Clinician: A Workflow Approach to Dysphagia Care Across Settings

Presented by: Theresa Richard, MA, CCC-SLP, BCS-S

🗓️ February 25th, 2026 | 7-8:30 PM EST

After this course, you will be able to…

  • Identify at least three common workflow bottlenecks in dysphagia care across inpatient, skilled nursing, and outpatient/private practice settings that contribute to delayed evaluation, prolonged diet restrictions, and continuity of care failures.

  • Describe how systems-level tools, including electronic medical records (EMR) triggered consults, standardized screening protocols, referral pipelines, and clinical decision support that can be adapted across care settings to accelerate patient access to appropriate dysphagia management.

  • Apply a framework for evaluating your own facility’s or practice’s dysphagia workflow to identify one actionable quality improvement initiative they can lead.

LIVE Agenda:

7:00-7:05 PM EST: Welcome & Framing Why dysphagia inefficiency is a systems problem, not a clinician problem and why that’s true whether you work in a hospital, a skilled nursing facility (SNF), or private practice.

7:05-7:15 PM EST: The Workflow Breakdown Across Settings Where dysphagia care falls apart including referral delays in acute care, inconsistent screening in SNFs, fragmented continuity into outpatient, and the unique bottlenecks private practice speech-language pathologists (SLPs) face with physician referrals and insurance authorization.

7:15-7:25 PM EST: Faster Entry Into Care Electronic medical record (EMR) triggered consults and automatic referral pathways in hospitals and SNFs. Building referral pipelines and physician relationships in private practice. Priority identification systems for high-risk patients regardless of setting.

7:25-7:35 PM EST: Standardizing Assessments Without Cookie-Cutter Care Standardized clinical swallow evaluation (CSE) protocols and validated tools adapted for each setting. When the bedside is enough versus when instrumental assessment is needed. Fiberoptic Endoscopic Evaluation of Swallowing (FEES) as a workflow accelerator in hospitals, SNFs, and mobile/private practice models.

7:35-7:45 PM EST: Treatment Planning & Documentation at the Speed of Your Setting Integrating dysphagia care into interdisciplinary rounds in acute and SNF settings. Documentation optimization, smart phrases, and clinical decision support. Private practice considerations including efficient reporting that supports reimbursement and referral source communication.

7:45-7:55 PM EST: Transitions of Care-The Most Dangerous Gap- Why handoff failures between acute care, SNF, and outpatient are a patient safety issue. Auto-referral workflows, warm handoffs, and building the receiving end of the continuum, especially for SLPs in SNF and private practice catching what the hospital missed.

7:55-8:00 PM EST: From Frustration to Change- Plan, Do, Study, Act (PDSA) cycles and workflow mapping adapted for your setting whether you’re pitching to hospital administration, a SNF director of rehab, or building systems in your own practice.

8:00-8:30 PM EST: Q&A
